When enrollment is decided, a provider questionnaire will be sent. Physicians and mid-levels (APP – Advanced Practice Provider) are eligible to enroll and must meet the criteria. The UP-Credentialing Team is certified by the National Committee for Quality Assurance (NCQA), an organization that works to improve health care quality through programs and accreditation.
Our Credentialing and Plan Enrollment teams work together to ensure providers are credentialed and active with the Health Plans in a timely manner.
