When enrollment is decided, a provider questionnaire will be sent. Physicians and mid-levels (APP – Advanced Practice Provider) are eligible to enroll and must meet the criteria. The Credentialing process will usually take 45-60 days to complete. The UP-Credentialing Team is certified by the National Committee for Quality Assurance (NCQA), an organization that works to improve health care quality through programs and accreditation.
The Credentialing Team and Plan Enrollment Team will be in direct contact once credentialing begins. When credentialing is complete, and payment is made, the health plan contracts will be sent, and effective dates will be provided.
