33 United Physicians' Practices Designated As Blue Cross Patient-Centered Medical Homes (PCMH) |

Bingham Farms, MI - United Physicians today announced that Blue Cross Blue Shield of Michigan (BCBSM) has designated 33 United Physicians' primary care practices as Patient-Centered Medical Homes - a distinction that is earned through a demonstrated commitment and adoption of practices that promote better, more engaged physician-patient relationships that result in improved health and outcomes. "Our network of physicians has experienced tremendous gains in growth in this area. In fact, our 33 practices represent a 220% increase over the 10 practices that were PCMH designated in 2009," said Mike Williams, M.D., President of United Physicians. "This growth rate clearly outpaces the 67% increase in PCMH practices that BCBSM designated across the State of Michigan in the same time period." To become a PCMH designated practice, primary care physicians lead a professional healthcare team that tracks and monitors the patients' overall health. Teams make sure that all the patient's healthcare needs are being met by working through their healthcare team to coordinate patient's prescriptions, therapies, tests and visits to specialists.
