Pediatricians Recognized for High Quality Care

Two United Physicians' pediatric practices, Pediatric Specialists of Bloomfield Hills and Southwest Pediatrics, have been recognized for their achievements in patient care quality.  These practices were selected from over 350 primary care practices statewide participating in the Michigan Primary Care Transformation Project (MiPCT).  MiPCT is a five-year, statewide effort to develop the patient-centered medical home model with an emphasis on care management. Practices were assessed by MiPCT on whether patients received appropriate screenings and whether unnecessary emergency department visits and hospital admissions were avoided.  Pediatric Specialists of Bloomfield Hills was rated number one in the state and Southwest Pediatrics was rated number three.

RN Care Manager 
In collaboration with the primary care physician, proactively outreach and engage patients in the care management program. 
Conduct comprehensive assessments and screening to identify areas of risk that may impact the patient’s adherence to the treatment plan. 
Develop, monitor, and revise an individualized care plan based on the patient’s goals and assessment findings.  
Coordinate transitions of care and facilitate close monitoring of complex patients to prevent unnecessary utilization associated with acute exacerbations.  
Provide patient education and self-management support to reduce risk behaviors and promote quality outcomes.
Participate in weekly interdisciplinary team care conference and collaborate with team to develop care goals for resolving patient issues.
Document and bill for services per payer and regulatory guidelines. 
Serve as a positive ambassador for United Physicians in the practices and support initiatives leading to practice transformation.  
Adhere to the scope of practice for Registered Nurse per state regulatory guidelines.
Respect confidentiality of all persons and follows state, federal guidelines and organization policies and procedures.
Miscellaneous duties as assigned.
Graduate of accredited school of nursing, BSN preferred
Valid unrestricted State of MI Registered Nurse License required
Case Management Certification and /or Chronic Care Professional preferred 
Valid State of MI Drivers License
Minimum two years of case management, home health care, hospice, palliative care or related experience 
Excellent verbal and written communication skills   
Ability to work under minimal supervision 
Working knowledge of care coordination and community resources 
Experience with HEDIS quality measures
Proficient computer skills including Microsoft Office Suite, EMRs, and patient registry

Data Analyst I
Conduct detailed analysis and data-driven research to monitor physician performance and quality across a wide spectrum of various healthcare metrics and measures 
Produce and generate ad-hoc reporting from various internal and external data sources including data warehouse data and data files/reports from health plans
Act as a data integrator to develop reports and analyses by collecting, merging, and summarizing health plan data into consumable report deliverables
Develop, test and execute standard reporting processes (monthly, quarterly, annual) 
Write SQL scripts/queries for clinical and financial reports utilizing various data sources within the organization
Support departmental functions for producing provider and practice level reports, distributing health plan incentive projects/programs and responding to physician offices inquiries for specialized requests for information and reports. 
Partner with Information Technology Department to identify and troubleshoot data related issues as well as define and gather requirements and execute user acceptance testing for data warehousing and other system related projects 
Participate on special, major reporting and analytical initiatives as directed from executive leadership
Thoroughly document report designs, business analyses, query objects and metadata 
Bachelor degree in Finance, Accounting and/or Information Technology
Two to three (2-3) years experience in data analysis and reporting
Background in health care related date preferred
Knowledge of medical terminology and physician information preferred
Understanding of quantitative and qualitative reporting 
Experience with Crystal Report Writer or similar reporting software
Knowledge of relational databases
Proven experience with Microsoft Excel, Access and SQL
Demonstrated analytical and problem solving abilities
Ability to obtain requirements, design, build, test and execute report objects

Desktop Support Specialist
Perform Help Desk Support for all staff of organization, troubleshooting and resolving issues.
Support remote access to network via UP owned laptops.
Upgrade software with patches/upgrades as they are available.
Support desktop and servers in Windows 7 environment.
Maintain company’s existing hardware/software. 
Maintain laptops, workstations, phones, faxes, copiers and printers.
Maintain Inventory system in Track-it application for software/hardware.
Optimize utilization and management of all software licenses and assure compliance with all software licensing agreements.
Work closely with system users to understand and analyze specific business needs. 
Work some weekends to support weekend system upgrades database maintenance or security activities.
Work on Special Projects assigned by Director of IT Technical Services. 
Two years of experience, or equivalent combination of education and experience  
Experience in all of the above referenced areas
Must be enthusiastic and professional and have good communication skills
Prior Helpdesk experience preferred
Excellent communication and organization skills

Bingham Farms, MI—August 14, 2014—United Physicians members are setting a level of health care excellence in Michigan. Six member practices recently received Priority Health’s 2014 Quality Award. The awards, established by Priority Health in 2003, recognize physicians for providing exceptional patient care, and making health care more accessible and affordable in our communities.

134 United Physicians Practices Designated as Blue Cross Blue Shield of Michigan Patient-Centered Medical Homes

Bingham Farms, MI—July 24, 2014—United Physicians announced today that Blue Cross Blue Shield of Michigan has designated 134 United Physicians primary care practices representing 360 primary care physicians (PCPs) as Patient-Centered Medical Homes (PCMH). The designation period runs from July 1, 2014 through June 30, 2015.

Botsford Hospital joins UP’s Facility Census to exchange ADT information

United Physicians and Botsford Hospital have finalized a process for exchanging Admission, Discharge and Transfer information through UP’s Facility Census.  This partnership will enhance patient care coordination and reduce readmissions by providing participating physicians with timely information on attributed patients that have been hospitalized at Botsford.

The following are the nominees for the four open positions on the UP Board of Directors.  The terms of office will be for three years, commencing February 1, 2014 through January 31, 2017.

  • Edward Ruiz, M.D. (Incumbent) - Dr. Ruiz is a dermatologist with privileges at William Beaumont - Troy and Crittenton hospitals.  He has served on the United Physicians Board since 2011.  In   2011 he was appointed to the Corporate Services Committee and has served on Finance Committee and Executive Committee of the Board for the past two years.  Dr. Ruiz is dedicated to continuing his work to strengthen United Physicians in the changing healthcare landscape.

  • Charles Stroud, M.D. (Incumbent) - Dr. Stroud is an Orthopedic Surgeon with privileges at William Beaumont - Troy hospital.  He was appointed to the Board in 2013 to fill the unexpired term of a departing Board member.  He currently serves on the Performance Management & Credentialing Committee of the Board.

  • Seymour Baxter, M.D. (Incumbent) - Dr. Baxter is a psychiatrist with privileges at William Beaumont - Royal Oak hospital.  He has a long standing relationship with United Physicians and has worked in varying capacities in previous administrations.  Dr. Baxter currently sits as the Chairman of the Board and is a member of the Executive Committee and Performance Management & Credentialing Committee of the Board.

  • Karen Lockwood, M.D. - Dr. Lockwood is an internal medicine physician with privileges at William Beaumont - Royal Oak hospital.  She has previously served on the United Physicians Board and Corporate Services Committee.  Dr. Lockwood is interested in dedicating her time to helping United Physicians members succeed in the future as health care continues to evolve.

  • Peter Muller, M.D. - Dr. Muller is pediatrician with privileges at St. Joseph Mercy Hospital Oakland, William Beaumont Hospital - Royal Oak, William Beaumont Hospital - Troy and Children's Hospital of Michigan.  Dr. Muller has previously served on the United Physicians Board and Performance Management & Credentialing Committee as well as the UP Executive Committee.  Dr. Muller is interested in continuing to serve the members of United Physicians by dedicating his time and experience to Board related matters.
