Organized Systems of Care & Accountable Care Organizations |
|
The entire healthcare system in the United States is undergoing a monumental transformation from a volume-based system to a value-based system. One healthcare model that offers a great deal of promise to help promote the transition is the development of Organized Systems of Care (OSCs) - also known as Accountable Care Organizations (ACOs). OSCs and ACOs are intended to foster more collaborative, integrated care among a defined group of caregivers who are accountable for the quality, cost and overall care of a defined population of patients. Everybody, from the 1,500+ independent doctors who are members of United Physicians, to the largest hospital systems, are focused on transforming themselves for the future of healthcare and participation in an OCS/ACO. At United Physicians, our extensive experience and resources that support Patient Centered-Medical Home initiatives, numerous pay-for-performance programs, and Health Information Technology make our network well-suited for effective participation in an OSC/ACO. |
