United Physicians recognizes the responsibility and importance of safeguarding the privacy of patients, whose health and health care information, and of our physicians, whose personal and practice information, is furnished to, and maintained by UP and to protect the security and confidentiality of such information. UP employees have access to confidential information on a need to know basis, concerning patients, physicians and providers. All UP employees are required to maintain strict confidentiality of this information at all times.

All Protected Health Information (PHI) of patients will be kept pursuant to applicable regulations of the Health Insurance Portability and Accountability Act of 1996, as amended. This includes, but is not limited to:

  • Any information and/or identification regarding individual patients, physicians, providers or health plans.
  • Any data or information regarding UP’s policies or administration of programs, whether already in place or developed by a UP employee.
  • Written correspondence to/from UP including, but not limited to, letters and copies of information such as internal, administrative and/or outgoing correspondence.

PHI may not be disclosed without the authorization of the individual. Patients may approve or deny the release of identifiable medical or other information by UP except when such release is required by law.
